01 What is lactose intolerance?
Lactose intolerance means your body has trouble digesting lactose, the natural sugar found in milk and dairy products. It happens when the small intestine doesn't produce enough lactase — the enzyme that breaks lactose down into two simpler sugars, glucose and galactose, that your body can absorb.
When lactose isn't broken down, it travels undigested into the colon, where gut bacteria ferment it. That fermentation is what causes the classic symptoms: bloating, gas, cramping, and diarrhea, usually appearing thirty minutes to two hours after eating or drinking dairy.
Lactose intolerance is not the same as a milk allergy. An allergy involves the immune system reacting to milk protein; lactose intolerance is a digestive issue caused by a shortage of one enzyme.
عدم تحمل اللاكتوز يعني أن جسمك يواجه صعوبة في هضم اللاكتوز، وهو السكر الطبيعي الموجود في الحليب ومنتجات الألبان. يحدث ذلك عندما لا تُنتج الأمعاء الدقيقة كمية كافية من إنزيم اللاكتاز، وهو الإنزيم المسؤول عن تفكيك اللاكتوز إلى سكرين أبسط، الغلوكوز والغالاكتوز، يستطيع الجسم امتصاصهما.

عدم تحمل اللاكتوز ليس نفسه حساسية الحليب؛ فالحساسية تتعلق برد فعل الجهاز المناعي تجاه بروتين الحليب، بينما عدم تحمل اللاكتوز مشكلة هضمية سببها نقص في إنزيم واحد.
02 Why we develop lactose intolerance
There isn't a single cause — clinicians recognize four distinct types.
Primary lactose intolerance
The most common type. Everyone is born producing lactase, but in most people production naturally declines after early childhood as the body adapts to no longer needing large amounts of milk. The decline is genetically programmed, and its timing varies — some people notice symptoms as teenagers, others not until their thirties or later.
Secondary lactose intolerance
A temporary drop in lactase caused by damage to the small intestine — often from a gut infection, celiac disease, Crohn's disease, or small intestinal bacterial overgrowth (SIBO). Lactase sits on the surface of the intestinal lining, so it's usually the first enzyme affected by injury and the last to recover. Tolerance often improves once the underlying condition is treated and the gut heals.
Developmental lactase deficiency
Seen in babies born prematurely, since lactase production only ramps up in the third trimester. Usually temporary.
Congenital lactase deficiency
An extremely rare inherited condition in which the small intestine produces little to no lactase from birth.
Lactose intolerance also isn't evenly distributed across populations. It's more common in people of African, Asian, Hispanic, and Mediterranean descent, and less common in those of Northern European descent — largely because of a genetic mutation that lets lactase production continue into adulthood in some populations.

عدم تحمل اللاكتوز الأولي
النوع الأكثر شيوعًا. يولد جميع الأشخاص قادرين على إنتاج اللاكتاز، لكن الإنتاج يتراجع بشكل طبيعي لدى معظم الناس بعد مرحلة الطفولة المبكرة، مع تكيّف الجسم مع عدم الحاجة إلى كميات كبيرة من الحليب. هذا التراجع مبرمج وراثيًا، ويختلف توقيته من شخص لآخر؛ فبعضهم يلاحظ الأعراض في سن المراهقة، وآخرون لا يلاحظونها إلا في الثلاثينيات أو ما بعدها.
عدم تحمل اللاكتوز الثانوي
انخفاض مؤقت في إنزيم اللاكتاز بسبب تلف في الأمعاء الدقيقة، غالبًا نتيجة عدوى معوية، أو الداء الزلاقي، أو داء كرون، أو فرط نمو البكتيريا في الأمعاء الدقيقة (SIBO). ولأن اللاكتاز يوجد على سطح بطانة الأمعاء، فهو عادةً أول إنزيم يتأثر بالإصابة وآخر إنزيم يتعافى. وغالبًا ما يتحسن التحمل بعد علاج السبب الأساسي وشفاء بطانة الأمعاء.
نقص اللاكتاز التطوري
يظهر لدى الأطفال المولودين قبل أوانهم، لأن إنتاج اللاكتاز لا يكتمل إلا في الثلث الأخير من الحمل. وهو عادةً مؤقت.
نقص اللاكتاز الخلقي
حالة وراثية نادرة جدًا، تُنتج فيها الأمعاء الدقيقة كمية ضئيلة جدًا من اللاكتاز أو لا تنتجه إطلاقًا منذ الولادة.
كما أن عدم تحمل اللاكتوز غير موزّع بالتساوي بين الشعوب؛ فهو أكثر شيوعًا لدى الأشخاص ذوي الأصول الأفريقية والآسيوية واللاتينية والمتوسطية، وأقل شيوعًا لدى ذوي الأصول الأوروبية الشمالية، ويعود ذلك بشكل كبير إلى طفرة وراثية تسمح باستمرار إنتاج اللاكتاز حتى مرحلة البلوغ لدى بعض الشعوب.
